Please provide a 250 word summary of the surgical video being submitted.: This video details the presentation of a patient with refractory heart failure secondary severe, secondary MR in the setting of a Star Edwards aortic valve and densely calcified anterior mitral leaflet and annulus. Given the poor visibility of the anterior mitral annulus and its severe calcification a commando procedure is performed. The steps of the procedure are detailed including exposure, valve implantation and patch reconstruction of the mitral annulus, dome of the left atrium, aortomitral curtain, aortic annulus and ascending aorta and closure.
